To become a member of the RSCDS Twin Cities Branch, please print this form, fill it out, and submit it to the branch treasurer, Theresa Livingston, along with a check for membership dues made out to RSCDS Twin Cities Branch. Please do not submit a check without a completed form accompanying it. You may mail the completed to Theresa Livingston at 1659 Chamber Street, St. Paul, MN 55106.

NOTE: The membership year runs from July 1 to June 30.
